Meningeal Cyst Classification

Nabors Classification

The Nabors classification organizes spinal meningeal cysts into three types based on their relationship to the nerve roots and meninges:

Type I — Extradural Meningeal Cyst (No Nerve Root Fibers)

Type II — Extradural Meningeal Cyst (With Nerve Root Fibers)

Type III — Intradural Meningeal Cyst

Clinical Pearl

The Nabors classification is clinically useful because it guides management: Type I (extradural, no nerve roots) and Type III (intradural) can be surgically excised or fenestrated when symptomatic. Type II (Tarlov cysts with nerve fibers) are more complex to treat because the nerve root is incorporated into the cyst wall, and surgery risks nerve injury.
